Why so many forms?
State and federal law requires that certain information be contained in each patient's file. I have attempted to make the paperwork as painless as possible by making my forms available on line. I do have copies of all the forms available at my office if you prefer to fill them out when you come in for you appointment (please come about 15 minutes early).
